Biography:
Dr. Wei-Chih Chen is an attending physician in the Department of Chest Medicine at Taipei Veterans General Hospital since 2013. He also serves as Director of the Respiratory Care Center at the hospital and is an assistant professor at National Yang Ming Chiao Tung University. Dr. Chen completed his MD at National Yang Ming University in 2006 and pursued residency and fellowship training in Department of Respiratory Therapy and Chest Medicine at Taipei Veterans General Hospital. He earned his PhD in Institute of Emergency and Critical Care Medicine at National Yang Ming Chiao Tung University .
His research interests mainly focused on pulmonary infection, mechanical ventilation, ARDS, and pulmonary fibrosis. He has more than 60 publications and numerous awards, including the best oral presentation from the Taiwan Society of Pulmonary and Critical Care Medicine. He has also been recognized as a best teacher multiple times by National Yang Ming Chiao Tung University and Taipei Veterans General Hospital.
Dr. Chen is actively involved in pulmonary and critical care societies and he is currently serving as the Deputy Secretary General of the Taiwan Society of Critical Care Medicine.
Severe acidemia, a condition that challenges homeostatic mechanisms and poses life-threatening risks when pH drops below critical thresholds. Typical acidemia scenarios, including septic shock, chronic obstructive pulmonary disease (COPD), and diabetic ketoacidosis are discussed. Each medical condition requires distinct interventions such as insulin, fluids, and sodium bicarbonate administration. The use of sodium bicarbonate is critically examined, emphasizing its potential risks, including intracellular acidosis, hypocalcemia, and hypervolemia, despite its role in attempting to correct severe metabolic acidosis.
The importance of early nephrologist involvement was emphasized again, particularly in cases of lactic acidosis, acute kidney injury (AKI), and acute respiratory distress syndrome (ARDS). The timing of renal replacement therapy (RRT) and its role in preventing respiratory muscle exhaustion and managing fluid overload are also discussed.
Sodium bicarbonate’s role in lactic and respiratory acidosis is debated, with data from randomized controlled trials showing mixed outcomes, especially regarding its effect on mortality in patients with septic shock and AKI. The complexities of decision-making process in these critically ill patients and the use of individualized treatment approaches while closely coordinating with nephrology are also discussed.
In summary, severe acidemia and hypoxemia have several physiologic challenges. Tailored treatment options and collaborative strategies are necessary to better manage severe acidemia and multiorgan failure in critical care settings.

Neuroprognostication of traumatic brain injury-Prof. Cherylee W. J. Chang (U.S.A)
Following severe traumatic brain injury (TBI), patients may recover over a prolonged period. Providers are uncertain as to the degree of neurological recovery and timeframe. Early negative prognostication and recommendation of withdrawal of life-sustaining therapy may not allow the patient adequate recovery time. However, when there is little chance that the patient will recover to a quality of life that the patient would find acceptable, there is a strong impetus for accurate prognostication given considerations such as prolonged family distress, resource utilization, and financial implications associated with a long period of aggressive medical support.
This presentation will review previous tools including the clinical examination, cerebral imaging, various predictive models, e.g. CRASH and IMPACT, and artificial intelligence to predict outcome following TBI.
After this presentation, the audience will be aware of the current research efforts and tools available exploring the neuroprognostication of TBI.

Vasopressors can cause significant tissue injury if extravasation occurs through peripheral IV lines. While central administration of vasopressors has become standard practice in recent decades, it can lead to delays in vasopressor initiation and complications related to catheter insertion. However, emerging data suggests that peripheral administration of vasopressors may be safe. Implementing a safety protocol that includes frequent monitoring, an extravasation management plan, limiting the dose and duration of vasopressor use when possible, utilizing large IVs in the forearm or upper arm, and confirming needle placement with ultrasound may help mitigate severe complications. Despite these insights, practices regarding peripheral vasopressor administration vary widely among hospitals, highlighting the need for updated policies to ensure its safe use.

Dr. Thomas Tao-Min Huang MD, a distinguished nephrologist, is an Attending Physician in the Department of Internal Medicine (Nephrology Division) at National Taiwan University Hospital (NTUH) and College of Medicine. With his tenure at NTUH starting in 2002 as a Rotation Intern, Dr. Huang has held multiple esteemed positions, including Chief of the Center for Quality Management and the Center for Dialysis at NTUH Yunlin Branch. Biography:
Vinay Nadkarni MD, MS is a Professor at the Children’s Hospital of Philadelphia (CHOP),
and founding director for the CHOP Academy for Resuscitation of Children, Center for Pediatric
Resuscitation and the CHOP Center for Simulation, Advanced Education and Innovation. Dr.
Nadkarni is an active leader of some of the most influential international societies and organizations
in the field of critical care and resuscitation science: the American Heart Association, ILCOR (the
International Liaison Committee on Resuscitation), WFPICCS (the World Federation of Pediatric
Intensive and Critical Care Societies), and served as the President of the Society of Critical Care
Medicine 2023. Dr. Nadkarni has authored more than 750 peer-reviewed manuscripts and 35 book
chapters related to the practice of critical care. Over the course of his career, he has mentored
more than 150 postdoctoral fellows and physician-scientists, culminating in recognition by the
SCCM Asmund S. Laerdal Award, American Academy of Pediatrics Critical Care Lifetime
Distinguished Career Award, the International Liaison Committee on Resuscitation (ILCOR) Giant’s
award, the AHA Lifetime Achievement Award for Resuscitation Science, the Citizen CPR
Foundation Hans Dahll Award, and the AHA Ron Haddock International Impact Award.

East Asia has the highest number of reported cases of ECPR (Y-A ECMO) in the world. The SAVE J II study, the world's largest (36 centers, 1500 patients) retrospective observational study in Japan reported in 2022, showed a good neurological outcome of 14.1 % (16.7 % for shockable rhythm). Sub-analysis of SAVEJ II study shows the important information about inclusion criteria for EPCR like low flow time and sign of life, etc. In Japan, a multicenter, randomized, controlled trial will be initiated with a trained team on selected cases, where the efficacy of therapeutic hypothermia in addition to ECPR will be evaluated.

fellowship in The Alfred Hospital Intensive Care Unit, a quaternary referral centre in Australia.
He has a strong interest in haemodynamic monitoring, mechanical circulatory support, post-cardiac arrest
management and critical care ultrasonography, in which he has been leading educational efforts both locally
and in the region. He is also the Singapore National Investigator for a number of multi-centre trials in Cardiac
Critical Care. On the educational front, he is the coordinator for undergraduate medical education in
Cardiology and also serves in the Internal Medicine Residency Programme, Cardiology Senior Residency
Programme and Intensive Care Medicine Subspecialty Training Committee for post-graduate medical training
and has been awarded the Best Teacher and Top Ten Teacher Awards on many occasions.
He is currently Chair of the Singapore National Post-Cardiac Arrest and Survivorship (PCAS) Workgroup,
President of the Society of Intensive Care Medicine (Singapore), Board Member of the Chapter of Intensivists
in the Academy of Medicine, Singapore and Adjunct Associate Professor with the National University of
Singapore Yong Loo Lin School of Medicine.
The right ventricle is a humble chamber that serves as a receptacle for systemic venous return and
pumps blood into the low pressure and highly compliant pulmonary circulation. It does its job dutifully
and seldom causes issues. However, when the right ventricle fails, it will give rise to a series of
problems that will need to be recognised and addressed given its difference from the left heart.
Hypoxaemia and hypercapnia are often present in ARDS and interventions to optimise oxygenation
and ventilation may not always be successful and may be associated with haemodynamic instability.
It is important to aware of important heart-lung interactions and to remember the intricate role of the
right ventricle in cardio-respiratory physiology. In this talk, we will review the diagnosis of acute cor
pulmonale and focus on the intricacies in the optimisation of right ventricular preload, contractility and
afterload.

Biography:
Dr. Chien-Chang Lee, an expert in both information technology and medicine, has served as the Chief
Information Officer (CIO) for Taiwan's Ministry of Health and Welfare (MOHW) since January 16, 2024.
Before taking on this role, he was a Professor of Emergency Medicine and Deputy Director of the Center for
Intelligent Healthcare at National Taiwan University (NTU) Hospital. Dr. Lee completed his medical education
and residency at NTU and later earned a doctorate in health data science from Harvard University.
As CIO, Dr. Lee oversees IT systems and security for more than 5,300 staff members, develops IT policies,
and manages significant health IT investments to enhance public health in Taiwan. In his prior role at NTU, he
led the integration of AI into clinical practice, focusing on policy development, innovation, and risk
management.
Dr. Lee’s research spans translational medicine, big data epidemiology, and AI in healthcare. His
groundbreaking work on fluoroquinolone antibiotics resulted in regulatory changes by the FDA and updates to
prescribing guidelines worldwide. Dr. Lee also received the Gold Medal of The Edison Awards in the
"Cutting-Edge Tools" subcategory within the Engineering and Robotics category for his development of
HoloXMed, a technology designed for educational and diagnostic applications in medical practice. From 2021
to 2023, Stanford University recognized him as one of the top 2% of scientists worldwide, a testament to his

Assistant Professor James Yao-Ming Shih is the department chief and senior attending of the Acute Trauma & Surgical Critical Care Unit, Fu Jen Catholic University Hospital in New Taipei City, Taiwan.
Dr. Shih was a Bachelor graduate in both Biochemistry and Microbiology from the University of Maryland, USA in 1994. He later pursuit his interest in medicine in the Philippines and graduated with his MD from the Fatima College of Medicine in 2001. After undergoing residency trainings in general surgery and cardiovascular surgery, he took up fellowships in vascular surgery, acute trauma care, and surgical critical care medicine at Cathay General Hospital, Taipei City, Taiwan. In 2017, he obtained his PhD in Clinical Nutrition from the Taipei Medical University and undertook short-term clinical fellowships in surgical intensive care and nutrition at Klinikum St. Georg, Leipzig, Germany and at Rabin Medical Centre, Petah Tikva, Israel. In 2018, he received his assistant professor status in the Institute of Human Metabolism & Obesity at Taipei Medical University, Taipei, Taiwan.
Dr. Shih is board certified in cardiovascular surgery, trauma critical care and surgical intensive care medicine. His area of practice includes post-surgical and trauma critical care, perioperative acute heart failure with ECMO support system, E-CPR and TTM and metabolic care bundles for neuroinflammation modulation, organ support systems with VV-ECMO and CRRT, adult critical care nutrition including TPN implementation, and elderly nutrition. His interests are in disease metabolism and metabolic approach (metabolic care) to the deranged metabolic stress associated with organ failures during the acute phase of illness.
Over the past decade, we have gained a deeper understanding of how caloric and protein needs fluctuate during the clinical course of critical illness as patients improve or deteriorate. Critical illness, whether triggered by infection (e.g., septic shock) or non-infectious causes (e.g., trauma or non-septic pancreatitis), is characterized by an inflammatory response and activation of the stress response, including the Hypothalamic-Pituitary-Adrenal (HPA) axis and the release of counter-regulatory hormones such as noradrenaline, glucagon, and cortisol, along with the onset of insulin resistance. These responses result in altered metabolism, including hyperglycaemia, hypermetabolism, and accelerated catabolism.
Consequently, our approach has evolved beyond simply calculating caloric and protein targets at a specific point in time. We now recognize the importance of phase-specific caloric and protein requirements, with adjustments made throughout the different stages of illness. For example, nutritional demands increase progressively from the acute phase through to the late phase of illness, continuing into the recovery phase or the persistent inflammatory/immunosuppressive catabolic syndrome (PICS) phase.
However, current clinical biomarkers are insufficient to precisely delineate these phases, complicating the application of tailored, phase-driven nutritional therapy. Moreover, our current understanding of metabolomic profiling in critically ill patients remains limited, and the process is time-consuming. This makes the implementation of point-of-care metabolomic profiling a significant challenge in the modern management of nutrition therapy for critically ill patients.

Medicine, National Taiwan University in 2012. His clinical and research interests include
microcirculation, sepsis, blood purification, extracorporeal life support, therapeutic

Dr. Lin got his degree from China Medical University in Taichung Taiwan. After navy military service, he

received full specialty training in emergency medicine in Taipei. After specialty training, he went to WISER
and Southern Illinois University to learn simulation education. He has been in the emergency medicine field
for over 20 years. He got a Bioinformatic PhD degree from Taipei Medical University. During his service in
emergency medicine, he found that simulation is a powerful and efficient tool for education. Therefore he
devoted himself to the use of simulation in training medical staff for 14 years. Currently he is the attending
physician in Taipei Medical University Shuang Ho Hospital and he is also the director of clinical skill center
and the Director of Center for Education in Medical Simulation at Taipei Medical University which is SSH
accredited in Teaching and Assessment. He is the Board Member of Pacific Asia Society for Simulation in

My work continued with the AHA working on Get with the Guidelines projects, as well as community cardiovascular projects around cholesterol and hypertension management before transitioning to the Emergency Cardiovascular Care Science team.
Through my work with ECC I have had extensive experience in guidelines development, scientific statement management, CPR education development and community implementation of CPR training. Additionally, I supported ILCOR in their evaluation and development of consensus of science and treatment recommendations.
Nearly a year ago I transitioned to being the Director of Science for Adult Healthcare programs for the American Red Cross. I work closely with the Red Cross science volunteers to review ILCOR science and develop the American Red Cross guidelines and courses.
Community CPR Implementation in the United States
For decades the American Red Cross has delivered lifeguard training across the country in the effort to reduce the needless deaths from drowning. Based on that experience Red Cross has launched a community CPR initiative based on community cardiac arrest data to deliver needed CPR training. This presentation highlights some of the strategies that are being pulled for our work in lifeguarding and applied to community CPR training.

Dr. Harnod is the honorary Secretary Generalof Taiwan Society of Emergency & Critical Care Medicine (TSECCM), Taiwan, ROC. He is working as Associate Professor at Department of Emergency and Critical Care Medicine of Taipei Medical University Hospital SHH hospital.
After graduating from Taipei Medical University, Taiwan in 1995, he completed his residency training in Shin Kong Memorial Hospital, Taiwan and got the Board of Emergency in 1998. He was a clinical research fellow in the Department of Emergency at University of Alberta Hospital at 1999, in the Department of Emergency & Toxicology at Bellevue Hospital at 2000. He received his Ph.D degree in Medical Sciences in 2014 from the National Taiwan University. He was the Director of Department of Emergency and Critical Care Medicine, Fu Jen Catholic University Hospital, Taiwan from 2017~2019.
Dr. Harnod is not only specialized as an intensivist but also an emergency physician. He is also the instructors of critical care medicine and Advanced Cardiac Life Support (ACLS), Advanced Pediatric Life support (APLS), Advanced Intoxication Life support (AILS). Currently, Dr. Harnod is the honorary Secretary General of Taiwan Society of Emergency & Critical Care Medicine (TSECCM), He is also the Honorary President of the Taiwan Emergency Medical Technician Association (TEMTA). He has more than 20 publications of original articles, case reports, and review papers being published in peer review journals covering research fields of Emergency and Critical Care Medicine, Trauma and Stroke.

Neurointensive care is necessary to prevent secondary brain damage in comatose patients after return of spontaneous circulation. However, there are no neurointensive therapies that have been reported to be effective in randomized controlled trials, or the results are conflicting. In particular, discrepancies in temperature management have been pointed out as a cause of the mixed results of randomized controlled trials, and high quality temperature management has been proposed as a strategy to overcome these discrepancies. High-quality temperature management can be read as high-quality neurointensive care.
Another reason for the mixed results of the randomized controlled trials is that the severity of the patients included ranged from mild to the most severe. That is, in addition to patients with moderate severity of illness who should be eligible for the therapy, there are patients who are too mild to benefit from temperature management therapy and patients with the most severe illnesses who will not benefit from any therapy.
Although the selection of patients of moderate severity who should be indicated should originally be based on the severity of brain damage, in reality, patients are often selected based on systemic parameters. However, it has not been reported that high quality temperature control therapy, i.e., neurointensive care, is effective when selected based on systemic parameters. In the future, it will be necessary to select patients according to the severity of brain damage and prove efficacy in a small number of randomized controlled trials.

Dr. Ting-Tse Lin is a highly respected cardiologist and Associate Professor at National Taiwan University College of Medicine. Currently, he serves as an Attending Physician in the Internal Medicine Division of Cardiology at National Taiwan University Hospital, a position he has held since March 2022. He previously directed the Cardiovascular Center at National Taiwan University Hospital Hsinchu Branch and played a pivotal role in cardiovascular care as an Attending Physician there from 2013 to 2022. Dr. Lin also held the role of Deputy Secretary-General at the Taiwan Society of Cardiology from 2016 to 2020. His extensive training includes a fellowship in cardiovascular electrophysiology at the University of Tsukuba Hospital in Japan and residency training in internal medicine and cardiology at National Taiwan University Hospital. Dr. Lin’s research interests focus on heart failure, atrial fibrillation, and cardiovascular imaging, with numerous publications in prestigious medical journals. His clinical and academic achievements have made him a leading figure in cardiology in Taiwan.
Electrical storm (ES) and malignant arrhythmias pose a serious threat to patients with structural heart disease, significantly increasing mortality rates. ES, characterized by recurrent ventricular arrhythmias within a short period, is a critical clinical situation that requires immediate intervention. Despite advancements in pharmacological and interventional therapies, managing ES remains a challenge due to its complex pathophysiology and high recurrence risk.
This presentation will discuss the latest pharmacological approaches, including the use of antiarrhythmic agents such as amiodarone, beta-blockers, and novel ion channel modulators. We will highlight the efficacy and limitations of these therapies, particularly in acute ES episodes and long-term prevention of arrhythmic events. Additionally, we will explore interventional therapies, such as catheter ablation, which has emerged as an essential strategy in reducing ES recurrences by targeting the arrhythmogenic substrate. Special attention will be given to the role of implantable cardioverter-defibrillators (ICDs), as well as advanced techniques like stereotactic body radiation therapy for arrhythmia management.

Biography:
Wei-An Chang, M.D., Ph.D., is an attending physician in the Division of Pulmonary and Critical Care
Medicine at Kaohsiung Medical University Chung-Ho Memorial Hospital, Taiwan. Dr. Chang earned
his medical degree from Kaohsiung Medical University in 2005, followed by a Master’s degree in
Clinical Medicine in 2014 and a Ph.D. in Clinical Medicine in 2020 from the same institution.
His extensive training in internal medicine includes a residency, chief residency, and a focus on
pulmonary and critical care medicine.
Dr. Chang is board-certified in internal medicine, pulmonary and critical care medicine, chest
ultrasonography, and critical care medicine. His clinical expertise includes intensive care medicine,
bronchoscopy interventions,and the placement of peripherally inserted central catheters (PICC) in
intensive care units.
In addition to his clinical work, Dr. Chang is also involved in research, focusing on
next-generation sequencing (NGS)studies of critical care, respiratory diseases, lung cancer,
and pulmonary fibrosis.

Associate Professor Chia Yew Woon is a Senior Consultant Cardiologist and Intensivist and Director of the
Cardiac Intensive Care Unit in Tan Tock Seng Hospital, Singapore. He is one of the few physicians who are
dually trained and accredited in both Cardiology and Intensive Care Medicine, having completed his
fellowship in The Alfred Hospital Intensive Care Unit, a quaternary referral centre in Australia.
He has a strong interest in haemodynamic monitoring, mechanical circulatory support, post-cardiac arrest
management and critical care ultrasonography, in which he has been leading educational efforts both locally
and in the region. He is also the Singapore National Investigator for a number of multi-centre trials in Cardiac
Critical Care. On the educational front, he is the coordinator for undergraduate medical education in
Cardiology and also serves in the Internal Medicine Residency Programme, Cardiology Senior Residency
Programme and Intensive Care Medicine Subspecialty Training Committee for post-graduate medical training
and has been awarded the Best Teacher and Top Ten Teacher Awards on many occasions.
He is currently Chair of the Singapore National Post-Cardiac Arrest and Survivorship (PCAS) Workgroup,
President of the Society of Intensive Care Medicine (Singapore), Board Member of the Chapter of Intensivists
in the Academy of Medicine, Singapore and Adjunct Associate Professor with the National University of
Singapore Yong Loo Lin School of Medicine.

Breakthrough in Registry Data Management-Dr. Pui Ning Pauline Yeung/ Critical Care Medicine Unit, The University of Hong Kong
Pauline Yeung Ng is Clinical Assistant Professor of Critical Care Medicine at The University of Hong Kong. She completed a research fellowship at Massachusetts General Hospital in Boston where she specialized in outcomes research. Upon returning to Hong Kong, she has consolidated territory-wide efforts to utilize data from electronic health records to assess outcomes after ICU care. She has published in fields of extracorporeal life support, pharmacoepidemiology, and respiratory infections. Her research interests also include point-of-care hemodynamic assessment in the ICU. She is a keen ECMO educator and was the chief editor of the online “ECMO 101” course hosted on ELSO.org. She is currently Assistant Dean (Hospital Engagement) at the Faculty of Medicine.

As a member of the Asia-Pacific Extracorporeal Life Support Organization (APELSO), Dr. Pauline is advancing a collaborative project with TXP Medical to build a registry for patients with acute respiratory distress syndrome(ARDS).
